As an On-Call Nurse, you’ll be responsible for responding to patient/family phone calls and making visits as needed during weekday off hours and/or weekends. You’ll respond to the needs of hospice and homecare patients requiring 24 hour availability. The On-Call Nurse also conducts off hour patient admissions as required. Homeside Hospice

Homeside Hospice, a nurse owned and operated hospice, is looking for highly motivated, compassionate and energetic candidates for Per Diem On Call RN's, to cover Ocean, Monmouth, Middlesex, Union, Essex and Bergen counties.

Must have a current and valid license to practice professional nursing in the state of NJ. Preference of at least two (2) years of work experience as a Hospice nurse. Demonstrates professional competency, ability to assess and respond to the needs of patients and families in varied settings, and good organizational skills.

Responsible for but not limited to: after hours triage, admissions, pronouncements and emergency visits. Maintenance and restoration of health; prevents infection, accident and injury; performs an initial assessment and identifies problems for each patient upon admission to hospice; reassesses the patient’s nursing care needs on an ongoing, patient-specific basis and provides care which is consistent with the Plan of Care; and monitors the patient’s response to nursing care. Also responsible for teaching, supervising and counseling the patient, family members, and staff regarding nursing care and the patient’s needs, including other related problems of the patient at home.

Must be willing to travel multiple counties in NJ, will be compensated for paid tolls.

VNAHG is seeking compassionate and experienced Hospice Nurses for our Monmouth County area.  Responsible for providing skilled nursing care to patients with life threatening and life limiting illnesses and their families. Assesses and evaluates the needs of patients with life threatening and life limiting illnesses and their families and develops an appropriate plan of care. Identifies teaching needs and plans appropriate strategies for individuals and families. Collaborates with patient and family, other members of the disciplinary team and other health care providers in providing patient and family care. RN with Bachelor’s degree in Nursing preferred. 1+ year of supervised clinical experience as a registered nurse.  Excellent verbal and written communication skills.
